The Contra Costa Consortium is an umbrella network of clients, providers and
interested community members addressing the issues of HIV and AIDS in Contra Costa County.

Case Management and Primary Care make up the heart of the Contra Costa System of HIV Care. This system works with other systems, utilizing CARE Act funds as funding of last resort, to help individuals with HIV/AIDS access and stay in care.
